Residential Supports (including Individual and In-Home Supports)

The Support Coordinator is responsible for overseeing the day-to-day operations of one home located in Waltham that supports four adults with disabilities.

RESPONSIBILITIES:

  • Responsible for training and supervising all of the direct support professionals that work in the home.
  • Ensures that Individuals receive the supports needed to live as independently as possible, maintain optimal health, and have sufficient opportunities to engage in desired activities and interests at home and in the community.
  • Works with their team to ensure the home is safe, clean, and comfortable at all times.
  • All applicants must be 18 years of age or older with a valid driver’s license and reliable transportation.

QUALIFICATIONS:

  • High school diploma or GED required, Bachelor’s degree preferred.
  • Two or more years’ experience working with individuals with developmental disabilities required.
  • Supervisory or management experience strongly preferred.
  • MAP certification required or must become MAP certified within 6 months of hire.
  • Safety-Care certification required at some locations.

Experience using multiple systems (e.g., Log Notes, Staff Communication, Data Collection, Progress Notes, including Confidential, Medical, and ISP Records, Health Care and Financial Reporting System.
